How to refer a child to Chance UK
If you are a teacher or parent of a child who you would like to have a mentor to improve their self-esteem and help them find positive activities to enjoy, please read these documents:

Then fill in the referral form for your borough below and email it to either Ross Adams for Hackney or Islington:

Alternatively, you can download, print, fill in and post the form to us at Chance UK, London Fashion Centre, 89-93 Fonthill Road, London N4 3JH.
We will then assess this to ensure the child is suitable for Chance UK's mentoring programme, and get in touch with you after that.
